South Axholme Practice has a population of approximately 15,000 patients, operating across five sites located in Epworth, Belton, Haxey, Owston Ferry, and West Butterwick.
Due to our multi-site structure, the successful candidate must be able to travel between sites as required. We are a rural based practice in North Lincolnshire with close links to Scunthorpe, Gainsborough and Doncaster.

Qualifications
Essential
Registered Nurse with current registration with the Nursing and Midwifery Council (NMC)
Experience in primary care or community nursing
Competent in immunisations and cervical screening
Desirable
Independent Prescribing qualification
Experience with EMIS
Additional long-term condition training in Diabetes care and Management

Experience
Essential
Experience of working in a Primary Care setting
Desirable
Experience in managing Diabetic patient care
Disclosure and Barring Service Check
