Main Purpose of the Role
To provide safe and effective clinical triage for patients contacting or attending the GP practice with urgent or same-day healthcare needs. The post holder will assess patient symptoms, prioritise care appropriately, provide clinical advice, and direct patients to the most suitable clinician or service.
Key Responsibilities
Clinical Triage
- Undertake telephone triage assessments for patients presenting with acute health concerns
- Assess symptoms and identify red flags or deteriorating conditions
- Prioritise patients according to clinical urgency
- Provide evidence-based advice and self-care guidance where appropriate
- Escalate urgent or emergency cases promptly to GPs, emergency services, or senior clinicians
- Book patients into appropriate clinics or refer to alternative services including UTC, community pharmacy, or emergency care
Clinical Care
- Deliver nursing interventions within scope of practice
- Support same-day urgent care clinics where required
- Perform clinical observations and basic procedures as appropriate
- Administer medication in line with practice protocols and NMC standards
Communication & Patient Support
- Communicate clearly and compassionately with patients and carers
- Provide reassurance and health education
- Maintain accurate and contemporaneous electronic patient records
Team Working
- Work collaboratively with GP’s, ANP’s, Paramedics, Pharmacists, HCA’s, GPA’s, reception staff and multidisciplinary teams
- Participate in daily clinical huddles and handovers
- Support safe patient flow within the practice
Governance & Professional Responsibilities
- Maintain registration with the Nursing and Midwifery Council
- Adhere to safeguarding, confidentiality, and infection control policies
- Participate in audits, mandatory training, and continuing professional development
- Work in accordance with NHS and practice policies
Person Specification
Essential
- Registered Nurse with current NMC registration
- Strong communication and decision-making skills
- Ability to work under pressure and prioritise workload
- Competence in clinical assessment and recognising deterioration
- IT literacy and accurate record keeping
Desirable
- Experience in general practice, urgent care or community nursing
- Telephone triage experience
- Knowledge of primary care pathways
- Familiarity with SystmOne
- Minor illness experience
