Main Purpose of the Role

To provide safe and effective clinical triage for patients contacting or attending the GP practice with urgent or same-day healthcare needs. The post holder will assess patient symptoms, prioritise care appropriately, provide clinical advice, and direct patients to the most suitable clinician or service.

Key Responsibilities 

Clinical Triage

  • Undertake telephone triage assessments for patients presenting with acute health concerns
  • Assess symptoms and identify red flags or deteriorating conditions
  • Prioritise patients according to clinical urgency
  • Provide evidence-based advice and self-care guidance where appropriate
  • Escalate urgent or emergency cases promptly to GPs, emergency services, or senior clinicians
  • Book patients into appropriate clinics or refer to alternative services including UTC, community pharmacy, or emergency care

 Clinical Care

  • Deliver nursing interventions within scope of practice
  • Support same-day urgent care clinics where required
  • Perform clinical observations and basic procedures as appropriate
  • Administer medication in line with practice protocols and NMC standards

 Communication & Patient Support

  • Communicate clearly and compassionately with patients and carers
  • Provide reassurance and health education
  • Maintain accurate and contemporaneous electronic patient records

 Team Working

  • Work collaboratively with GP’s, ANP’s, Paramedics, Pharmacists, HCA’s, GPA’s, reception staff and multidisciplinary teams
  • Participate in daily clinical huddles and handovers
  • Support safe patient flow within the practice

 Governance & Professional Responsibilities

  • Maintain registration with the Nursing and Midwifery Council
  • Adhere to safeguarding, confidentiality, and infection control policies
  • Participate in audits, mandatory training, and continuing professional development
  • Work in accordance with NHS and practice policies

Person Specification

Essential

  • Registered Nurse with current NMC registration
  • Strong communication and decision-making skills
  • Ability to work under pressure and prioritise workload
  • Competence in clinical assessment and recognising deterioration
  • IT literacy and accurate record keeping

 Desirable

  • Experience in general practice, urgent care or community nursing
  • Telephone triage experience
  • Knowledge of primary care pathways
  • Familiarity with SystmOne
  • Minor illness experience
